5. DATA PROTECTION
HeartlandHealth safeguards your personal data in accordance with the Personal Data Protection Act (PDPA) and applicable healthcare regulations. Your data will only be used for the purposes stated.
4. CONSENT
By seeking care under the Schemes and/or using MediSave or subsidy benefits, you authorise the collection, use, and disclosure of your personal data as described in this Notice.
You may withdraw your consent by informing our clinic staff or PCN coordinators. However:
a.
Upon withdrawal, CHAS, SFL, VCDSS subsidies and MediSave claims will not apply, and
b.
You may no longer receive PCN care coordination services;
c.
Claims submitted before withdrawal that are pending processing or audit may be rejected, and any subsidies paid out may require reimbursement unless data sharing for such claims is allowed⁵.
3. PARTIES WITH ACCESS TO DATA
Your personal data may be shared with:
a.
The Ministry of Health (MOH), the Agency for Integrated Care (AIC), and other appointed parties³ for administration and audit of the Schemes;
b.
PCN Headquarters and associated multidisciplinary care teams, including care coordinators and nurse counsellors;
c.
Polyclinics, affiliated healthcare providers, and authorised service partners (e.g., IT vendors, laboratories) involved in your treatment and claims processing.
All parties are legally required to keep your data secure and confidential⁴.
2. COLLECTION AND USE OF DATA
When you seek care at HeartlandHealth clinics and utilise MediSave and/or receive subsidies under any of the Schemes, including CHAS, your personal data¹ and medical information, including but not limited to visit details, diagnoses, treatment provided, and subsidy claims, may be collected, used, and disclosed for the following purposes:
a.
Processing, verifying, and auditing subsidy claims;
b.
Coordinating and monitoring care under the PCN, including entry of relevant data into the Chronic Disease Registry²;
c.
Contacting you to verify visit or claims details or to facilitate care coordination;
d.
Assessing compliance of clinics and healthcare providers with the terms of the Schemes;
e.
Facilitating administration, review, and improvement of patient care, healthcare schemes, and public health policies;
f.
Submitting relevant data to the National Electronic Health Record (NEHR) and National Immunisation Registry (NIR) to support continuity of care.
1. PURPOSE
This Notice informs patients of the collection, use, and disclosure of personal data under the Community Health Assist Scheme (CHAS), Chronic Disease Management Programme (CDMP), Screen for Life (SFL), Vaccination & Childhood Developmental Screening Subsidy Scheme (VCDSS), and the Primary Care Networks (PCN) Scheme (collectively, the “Schemes”).

² Chronic conditions covered under PCN/CDMP refer to those listed under the Chronic Disease Management Programme (CDMP); relevant data may include earlier visits to support long‑term management.
